III.1.3.2) List and brief description of selection criteria Het UEA, waarmee de ondernemer verklaart dat hij voldoet aan de onderstaande selectiecriteria: 1. registratie bij FAGG voor de aangeboden medische hulpmiddelen; 2. voor elk van de aangeboden toestellen dient een CE certificaat volgens de Europese richtlijn Medische Hulpmiddelen 93/42/EEC en IEC60601 attest aan de offerte toegevoegd te worden; 3. een lijst van referenties in vergelijkbare projecten qua financiële omvang, complexiteit en configuratie, bij voorkeur in Belgische ziekenhuizen. Gelieve ook een mogelijke contactpersoon + coördinaten van deze referenties aan te geven. III.1.3.3) Minimum level(s) of standards possibly required 1. Certificaat FAGG betreffende de notificatie van distributie en/of uitvoer van medische hulpmiddelen art. 10 bis van het KB 18/03/1999 betreffende de medische hulpmiddelen, waarin de desbetreffende producten en/of fabrikanten aangaande dit dossier vermeld staan; 2. De attesten moeten per type toestel worden toegevoegd aan de offerte; 3. Nederlandstalige customer service.
