The service includes daily, weekly, monthly and periodical activities (general cleaning). II.1.6) This contract is divided into lots: no II.1.4) Short description Service procurement from cleaning and hygiene maintenance service for the following facilities of PHI Healthcare Center of Skopje CO Skopje: Bit Pazar Polyclinic, Chair Polyclinic, Bukuresht Polyclinic, Jane Sandanski Polyclinic and Gjorce Petrov Polyclinic. The service includes daily, weekly, monthly and periodical activities (general cleaning). II.2.5) Award criteria: Cost/Price Value excluding VAT: MKD 12300000.00 II.02.07) Duration in months 12 II.2.7.5) This contract is subject to renewal: no II.2.10) Variants will be accepted: no The procurement is related to a project and/or programme financed by European Union funds): no III.1.1) List and brief description of conditions In order to prove its personal condition the economic operator is obliged to submit the following documents: - Statement that in the past five years it was not rendered a final court judgment for committed crime pursuant to Article 88, paragraph 1 of the PPL - Confirmation by a competent organ that a bankruptcy procedure was not opened - Confirmation by a competent organ that a liquidation procedure was not opened - Confirmation for paid taxes, contributions and other public duties issued by a competent organ in the country where the economic operator was registered - Certificate from the Registry of penalties for crimes committed by legal persons that minor penalty ban on participation in public tender procedures, awarding contracts and agreements for public-private partnership has not been imposed on him - certificate from the Registry of penalties for crimes committed by legal persons that a minor penalty for temporary or permanent prohibition of performing a specific activity has not been imposed over him; - confirmation that a final judgment - prohibition of professional activity or duty, i.e. temporary prohibition for performing professional activity has not been imposed over him. Capacity for performing professional activity of economic operator: - Document for registered activity: In order to prove its capacity for performing professional activity, the economic operator must submit – Conformation for registered activity as an evidence that it is registered as a natural or legal entity for performing the activity related to the subject of the public procurement contract or an evidence that it is a member of a relevant professional association in accordance to the regulation of the country where it is registered. III.1.2) Economic and financial standing III.1.2.2) Selection criteria as stated in the procurement documents III.1.3) Technical and professional ability III.1.3.1.2) Selection criteria as stated in the procurement documents III.2.2) Contract performance conditions Quality systems standards: In order to qualify as capable for performing the subject public procurement contract from the aspect of the quality system standards, the economic operator should meet the following standards: - The economic operator should have implemented the quality management system standard ISO 9001: 2015 or equivalent. The economic operator should have implemented the occupational safety management standard ISO 45001:2018 or equivalent. The economic operator shall prove the meeting of the standards by submitting the Certificate for quality management system ISO 9001:2015 or equivalent issued by an accredited institution and Certificate for occupational safety management ISO 45001:2018 or equivalent issued by an accredited institution.
